The registration fee for delegates includes:

• Admission to the scientific sessions
• Lunch & Coffee break from Friday, October 20to Tuesday, October 24
• Congress kit
• Welcome Cocktail
• Social Dinner
• Farewell Dinner
• UEMS/CME credits

Please choose your workshops directly on the registration form, crossing INTERVENTIONAL or DIAGNOSTIC.
For organizing reasons, changes won’t be possible on site, so we kindly ask you to indicate your choice in advance.

Those who wish to participate in the Course are requested to:

Please note that AFTER October 6, 2006 registration will be possible only by fax or e-mail

CONFIRMATION OF YOUR REGISTRATION
A confirmation letter and the invoice for the Course and Accommodation fee will be sent after the registration form and full payment have been received. Please bring this confirmation to the Registration Desk.

CANCELLATION AND CHANGES
Alterations to your registration will only be accepted in writing, addressed to the Organizing Secretariat.
Reimbursement will be 50% of the amount paid for cancellations received in writing by the Organizing Secretariat before September 8, 2006.

Regretfully, after that date cancellations will not be entitled to any refund.

All refunds will be made AFTER the course.

INVITATION LETTER FOR VISA REQUIREMENTS
Please write to the Organizing Secretariat in order to receive copy of the VISA letter.
