What is covered by the Delegates Fee?
The registration fee includes admission to the Opening Ceremony and Welcome Reception, unlimited access to all scientific sessions, access to the exhibition, a full set of congress materials, refreshments and lunches from Sunday, June 13 to Wednesday, June 16, 2004.

Payment
Payment should be made by one of the following methods:

  1. By Bank Transfer
    Please transfer the payment to AISC & MGR - AIM Group.
    Bank details: Credito Emiliano, Via Andegari, 14 - 20121 Milano. Account nr. 55104/7 - CAB 01600 - ABI 3032. The payment transfer form should clearly state the name(s) of the delegate(s) and should clearly state the reference - IHHC 2004 - 4CC003.
  2. By Cheque
    The cheque should be made out in Euro to AISC & MGR - AIM Group and should be forwarded together with the registration form.
  3. By Credit Card
    All major credit cards are accepted (American Express, VISA, Eurocard/Master Card). Please enter the credit card number and expiration date in the appropriate space on the registration form.
  4. By Internet
    An “on line” registration form with payment by credit card in security certificate mode “Verisign” and “SSB” is available,>> click here <<

All fees are requested in Euro. Registration will be processed only if accompanied by total payment.

Confirmation of Registration
You will receive a written confirmation of your registration by the Organizing Secretariat.

Cancellation Policy
Notification of cancellation must be sent in writing to AISC & MGR - AIM Group. Cancellations will be accepted until April 30, 2004 with a refund of all prepaid fees except for a 30% administration charge. No refunds will be made for cancellations received after this date.
All approved refunds will be processed and issued 60 days after the Conference.

Name Changes
After April 30, 2004 sponsors who wish to make name changes will be charged a handling fee of Euro 40,00 (VAT 20% included) per name change. A new registration form for each replacement participant will be required before the name change can be processed. Individuals may make name changes only in exceptional cases. Written permission from the original participant will be required in order to process an individual name change.
